Their mission is to provide community and person-centred support and education to those impacted by dementia. The Alzheimer Society Southwest Partners provides social work support, counselling, client and public education, system navigation support and therapeutic recreation programs for individuals living with dementia and their care partners. They provide services throughout Elgin, Middlesex and Oxford counties. Programs support individuals with all forms of dementia, including Alzheimer’s disease and Mild Cognitive Impairment.
Offices are located in Elgin, Middlesex and Oxford.
